Employee's phone stored in pan directly on top of syrup packets on shelf in front of kitchen. Manager removed the phone from the area during this inspection. **Corrected On-Site**

Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Several employees handling food in kitchen with bracelets and/or watches on their wrists.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. 1. Area around nozzles of fountain machine in lobby soiled with mold-like substance. 2. Interior of cabinet below fountain machine in lobby and wrap around drain lines inside cabinet soiled with mold-like substance. **Repeat Violation**

Basic - Wiping cloth chlorine sanitizing solution not at proper minimum strength. Sanitizer bucket chlorine 0ppm. Manager had an employee dump and refill the bucket, then chlorine 50ppm. **Corrected On-Site**

Intermediate - Multiuse food-contact surface not smooth, free of breaks/cracks/chips/pits/crevices. Top of opening cracked near hinge of ice machine lid.

Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. No chlorine test kit available for chlorine sanitizer in sanitizer buckets.

Intermediate - Soap dispenser at handwash sink not working/unable to dispense soap. 1. Soap dispenser at handwash sink at cookline missing faceplate and unable to dispense soap. 2. Soap dispenser at handwash sink near drive through unable to dispense soap. The manager stated the dispenser may need a new battery and placed a hand pump soap bottle at the handwash sink during this inspection. **Corrective Action Taken**
