Basic - Floor tiles missing and/or in disrepair. Floor tiles in back kitchen near mop sink in poor repair. Observed wear at front floor tiles on make line. Observed porous tiles in back kitchen storage area. Floor around grease trap and triple sink with debris buildup.

Basic - Food stored on floor. Observed cases of chips near back hall stored on floor. Person in charge raised. Observed case of soda syrup stored on floor in back kitchen. Person in charge raised. **Corrected On-Site** **Repeat Violation**

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In walk in cooler roast beef, tuna, turkey, ham, cheese, steak 44-47F. Cheese in back of walk in cooler 43F. Ambient temperature of unit 44F. Person in charge states has been in and out of unit for prep this morning. Called for repair service during inspection. Intermediate - Manager or person in charge lacking proof of food manager certification. Manager on site during inspection has been manager for 18 years. No proof of CFM, only has Food Handler training at time of inspection.
