Basic - Bathroom facility not clean. Upon inspection, observed unknown brown substance present in one stall on floor and toilet seat in woman's bathroom at establishment.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Upon inspection, observed multiple ceiling tiles behind front counter area and in lobby to be soiled with dust. **Repeat Violation**

Basic - Damage to wall. Upon inspection, observed multiple areas with damage/rips in wallpaper inside the public areas to include main lobby and hallway areas of the establishment.

Basic - Floor tiles missing and/or in disrepair. Upon inspection, observed multiple floor tiles cracked/in disrepair in lobby and hallway areas at establishment.

Basic - Wall soiled with accumulated grease, food debris, and/or dust. Upon inspection, observed multiple walls to be soiled with unknown substances at establishment, to include public areas such as lobby and hallways.

Basic - Worn, torn and/or soiled floors/carpeting. Upon inspection, observed some carpeted areas heavily soiled/worn in public areas at establishment to include main lobby and hallways.

Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Upon inspection, no chemical test strips/kit available for sanitizer currently being used to sanitize in three compartment sink.

Intermediate - No soap provided at handwash sink. Upon inspection, observed hand soap dispenser missing in handicap stall of woman's bathroom.
