Inspector notes: Observed potentially hazardous food (mashed potatoes, dirty rice, and gravy on front serving line) not held at 135 degrees Fahrenheit or above. CA - Discus with PIC and he said that food was put on front serving line at 10:45 and it was 11:46, so had him reheat food but with CFSM arrived she had it discarded. Also discussed that light were out and heat elements not heating evenly. CFSM was going to have Maintenace look at unit and she also checked water levels during inspection.
