Inspector notes: Observed CFSM certificate not posted in public view upon arrival. Advised PIC The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times and shall be made available for inspection by the Health Authority. Advised PIC to post an unexpired CFSM certificate in public view.

Inspector notes: Observed food employee handle personal cell phone and began to handle clean utensil in order to prepare pizza dough. Also observed employee touch pants and proceeded to prepare dough with glove without washing hands. Advised PIC that Food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles. COS: PIC placed utensil at the 3-compartment sink and washed hands at the designated prep area.

Inspector notes: PIC could not demonstrate in a verifiable manner that food employees are/were aware of their responsibilities to report foodborne illnesses and symptoms. Advised PIC food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. Advised PIC to obtain an employee health policy and to sign conditional agreement forms.

Inspector notes: Observed an employee drink from a coffee cup without a lid and a straw near the prep area. Advised PIC Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed. Advised PIC to have employees drink from a cup with a lid and a straw at the designated area.
