Inspector notes: Observed an accumulation of ice on the floor of both walk in freezers. Observed leak from the bottom of the prep top cooler in the front food service. PIC discussed that the manager has contacted a technician to repair the leak from the prep top cooler. CA: Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections.

Inspector notes: Observed accumulation of dust and debris on a rack in the walk in cooler. Observed black slime that was easily wiped off with alcohol swab on the side of the ice machine near the lid. CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
