Inspector notes: Observer door of freezer chest in lobby in disrepair. Equipment and utensils shall be designed and constructed to be durable and to retain their characteristic qualities under normal use conditions. PIC advised to repair freezer door.

PIC advised to follow to clean all non-food contact surfaces of equipment until they are clean to sight and touch. CA. Observed glue fly trap hanging from ceiling in main kitchen with dead fruit flies and dead roaches on sticky trap on floor in chemical storage room. Also observed rodent droppings on floor in chemical storage room. PIC advised to remove traps with dead pests, clean chemical room to remove rodent droppings, have a licensed pest control officer treat facility for pests and provide pest control report.
