Inspector notes: Observed the can opener with a thick build up and metal shaving around the blade. Employee stated that facility does not use the can opener anymore. If it is not in use it must be removed and/or discarded or kept clean. Food contact surfaces must be clean to sight and touch.

Inspector notes: Observed some employees handling and preparing food items in the kitchen without a hair restraint. All employees must have hair fully restrained when in the kitchen.

Inspector notes: Observed the wiping cloths buckets at 0 PPM quaternary ammonium solution. Employee remade the solution and it was still at 0 PPM. After carefully looking at the chemical dispenser I noticed that the signage is incorrect and employees are filling the buckets with soap and water instead of sanitizer. Advised PIC to change the signage so that it will properly reflect the sanitizer and detergent. The buckets were remade to 200 PPM quaternary ammonium solution (COS).

Inspector notes: Observed the inspection report posted too low (below the counter) making it hard to see. Inspector notes: Observed some of the ceiling tile in the kitchen with yellow stains. The tile will need to be cleaned and/or replaced to make sure that it stays in good repair and clean. Observed the wall behind the 3-compartment sink with a black mold-like build up. Please clean and reseal the wall. Also, the floors underneath the 3-compartment has black build-up that must be cleaned.
