Inspector notes: PIC could not provide me with signed employee health reporting agreements. CA: The permit holder shall require food employees and conditional employees to report to the CFSM and person in charge, information about their health and activities as they relate to diseases that are transmissible through food. PIC stated that they will send me an email of the signed employee health reporting agreements. Discussed with PIC that they must have the signed agreements on site at all times.

15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Inspector notes: Observed two absorbent vinyl tiles on the ceiling in the main kitchen. CA: Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material. Discussed with PIC that they have 60 days to replace those tiles with a smooth, durable, and non absorbent material.

16B - plumbing installed; proper backflow devices
Inspector notes: Observed plumbing past the air gap for handwashing sink and three compartment sink possibly clogged. When the three compartment sink is emptied, the water rises above the rim of the air gap pipe and overflows on the floor. Discussed with PIC that they have 10 days to repair plumbing. Send completed work order to health inspector with email niasia.rouse@adph.ga.gov

Inspector notes: Observed back door in main kitchen with gap on the right side of the door. Observed back door near restrooms with air gap on the right side and the bottom of the door. CA: outer openings of a food service establishment shall be protected against the entry of insects and rodents by Solid, self-closing, tight-fitting doors. Discussed with PIC that they can get weather strips to cover the openings on both back doors. This will prevent pests from entering the establishment.
