Inspector notes: Observation: Observed chemical sanitizer in a sanitizer bucket just dispensed from the 3-compartment at 0 ppm quaternary ammonium. Corrective Action: Discussed required concentrations for quaternary ammonium with the PIC. A quaternary ammonium compound solution shall have a minimum temperature of 75ºF (24ºC), have a concentration as specified in DPH Rule 511-6-1-.07(6)(g) and as indicated by the manufacturer’s use directions included in the labeling, and be used only in water with 500 ppm hardness or less or in water having a hardness no greater than specified by the EPA-registered label use instructions. Inspector notes: Observation: Observed multiple potentially hazardous TCS food items not discarded by the manufacturer's expiration date (Yogurt 1/2/26, Milk Carton 12/8/25, Garlic Parmesan Sauce 3/17/25). Corrective Action: Discussed discarding TCS manufactured foods based on the provided expiration date with the PIC. Prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date or the sell-by date. The PIC discarded all expired foods.

Inspector notes: Observation: No chemical (quaternary ammonium) test kit provided when using chemical sanitizer at the 3-compartment sink. Corrective Action: Discussed testing frequency of chemical sanitizing solutions with the PIC. The facility must order and provide chemical test kids for chemical sanitizing solutions and keep them at the 3-compartment sink. A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided. This will be rechecked by the Health Department.

Inspector notes: Observation: Observed a heavy accumulation of food debris/ dust on the floor underneath multiple coolers, equipment, prep areas and the dish area. Corrective Action: Discussed cleaning methods and frequency of floors with the PIC. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. The PIC will have the area cleaned and will increase cleaning frequency. This will be rechecked by the Health Department.

Inspector notes: Observation: Observed the accumulation of dead roaches and other insects in glue traps and on the floor in the main kitchen area. Corrective Action: Discussed removing used insect traps with the PIC. Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests. The PIC will have the facility cleaned and the soiled insect traps removed. This will be rechecked by the Health Department.
