Inspector notes: Observed rice in dry storage to have a date label from 9/19/25. Observed multiple containers of cocoa powder to have expiration date of 11/22/22. Observed vanilla wafers to have a date label from 12/15/25. Observed spinach in walk-in cooler to have a date label from 12/15/25. COS: PIC discarded all expired items into the trash. CA: All opened food items can be held for a maximum of 7 days after the original container is opened. The day the original container is opened in the food service establishment shall be counted as Day 1, except, the day or date marked by the food service establishment may not exceed a manufacturer’s use by date if the manufacturer determined the use-by date based on food safety.

Inspector notes: Observed rack in walk-in cooler to have a sticker with black build-up on it adjacent to RTE lettuce and vegetables. COS: PIC removed the sticker and sanitized the surface. CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Inspector notes: Observed heavy mold-like build-up on ceiling tile over dish washing area. Observed a hole and structural bowing in the same ceiling tile in kitchen area. RCA: PIC shall have ceiling tile replaced. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
