Inspector notes: Observed raw meat prep area with sheetrock walls. Observed walls in non prep areas of kitchen made of sheet rock with chipped paint in various places. Materials for indoor floor, wall, and ceiling surfaces under conditions of normal use shall be: smooth, durable, and easily cleanable for areas where food service establishment operations are conducted; closely woven and easily cleanable carpet for carpeted areas; and nonabsorbent for areas subject to moisture such as food preparation areas, walk-in refrigerators, warewashing areas, toilet rooms, mobile food service unit servicing areas, and areas subject to flushing or spray cleaning methods. Inspector notes: Observed frp board pulled away from wall near back door. Observed some dust buildup along ceiling grid in kitchen. Observed some sagging ceiling tiles in kitchen area. All physical facilities shall be maintained in good repair and shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. PIC needs to repair any gaps in the wall and replace or reset any sagging ceiling tiles. PIC needs to clean facility and remove any dust or debris buildup.

Inspector notes: Observed front entrance and exit doors have gap between them through which light can be easily seen. Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: filling or closing holes and other gaps along floors, walls, and ceilings; closed, tight-fitting windows; and solid, self-closing, tight-fitting doors. PIC needs to add weather stripping to any door gaps.
