Inspector notes: Observed the ice bin in the main kitchen with accumulation of some type of black debris around the chute area.. PIC stated the ice is used for drinking and the ice bin is cleaned once a month. C/A In equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment: (I) At a frequency specified by the manufacturer; COS: Facility has 72 hours to correct the violation and send pictures to EHS stating the violation has been corrected Update: Manager sent pictures to EHS on 12/18/2015 and it shows the ice bin chute clean from any debris. Inspector notes: Observed two small plastic sanitizing trays on the counter with a concentration of sanitizer less than 200. Observed a large bucket stored under the counter at the front service area with a concentration below 200ppm (QUAT) C/A 2. Cloths in-use for wiping counters and other equipment surfaces shall be: (i) Held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n); and (ii) Laundered daily. COS: Sanitizer was changed and was tested at 400ppm
