Owner paid ML Business Consulting to handle their permitting, but health department did not receive funds. Manager was advised to contact the health department directly to pay for permit fees for restaurant, base of operation, and mobile unit.

Observed employee sleeping cot inside of the single-use articles and dry goods storage room. PIC was advised that employees are not allowed to lounge in areas where food and single use articles are stored. PIC removed the cot from the storage room.
