Inspector notes: Observed mold build-up along interior portions of bulk ice machine. Ice machine must be emptied, cleaned, and sanitized within 72 hours. Must increase cleaning frequency to a rate that precludes mold accumulations.

Inspector notes: Observed quat sanitizer not dispensing from 3-compartment sink. Level of chemical in container was very low and not being suctioned through line. Sanitizer spray bottles were also at 0 ppm as a result. Quat sanitizer must dispense at 150-400 ppm or as specified on manufacturers instructions. COS by replacing chemical. Sanitizer then dispensed at 400 ppm; sanitizer in spray bottles were also emptied and changed.
